TRAVEL GRANTS

Blank travel grants can be obtained from the MCFHT office or downloaded from the Ministry of Health and Long Term Care.

Please bring the travel grant to the clinic and place it in our drop box, located outside the front door of the clinic. 

OR, you can mail your completed travel grant to the clinic.

Before you leave or mail your documents, please ensure:

• The patient’s information is complete on the travel grant 

• The patient has signed the travel grant

• The specialist section on the travel grant is completed

• A stamped envelope with the appropriate address is included
